Jam Making Kits

I'd like to improve my diet & health. Any Help?

I'm 15 years old, female. I weight around 111 pounds and stand at 4 ft 11.
I pretty much eat the same things every single day and I really want to change my diet!
Everyday:
Breakfast: Half or maybe 1 slice of toast with jam, (I can't physically eat a lot in a morning)
Lunch: 1 ham sandwich, 1 apple and 1 kit-kat
Dinner: Some sort of family meal, usually with vegetables and always with some sort of meat (Don't really have much control over what I have for dinner)
Snacks: fruit, chocolate bars, toast, bits and bats
I NEVER eat crisps, mainly because I don't like them, I'm very fussy.
I drink quite a lot of apple and orange juice, to make up for the 5-a-day fruit and veggies
I drink at least a average bottle of water a day as well, I know I should drink more
I get around 3-4 hours of exercise a week when really i should be getting more.
So, how could I improve my diet and fitness? Thanks
You can do simple things like switch up white bread for wholemeal bread etc.
Remember 1hour a day of exercise is recommended.
And 8 glasses of water a day is recommended.
Don't drink apple and orange juice to 'make up for the 5-a-day'. Thats not the point. It is still good to drink these though. When a product says on it 'One of your 5 a day' it does mean JUST one and if you eat/drink more it does not count.
Eat a variety of food from each food group.
